To Purchase Using Health Insurance

1. Download the appropriate script form and/or letter of medical necessity for the product you are requesting, fill out the form and have your physician sign it.
2. Fax the script form and letter of medical necessity along with your insurance information to 1-866-716-9008. If you would rather call with your insurance information, please give us a call at 713-218-7959.
3. Once we receive your signed script form and any supporting documentation we will verify your insurance benefits and contact you within 48 hours.
